4. PART 147 Authenticated Document Letters with Air Agency Number
5. FAA / PSI Knowledge test results for each subject (General, Airframe, Powerplant)
6. Cash payments only
7. Re-test you must bring ALL previous 8610-2 Forms
8. Re-test < 90 days additional training certificate must be signed by trainer, name and certfication #
